UI print and online forms: You may apply online or download these forms from Graduate Admissions or the College of Graduate Studies:

• Application for Admission to the College of Graduate Studies. This application form is to be used by U.S. citizens and permanent resident aliens; international students must complete the international application for admission.
• Application for Readmission. Use this form if you are a graduate student and have not taken a course during the past 2 years.
• Appointment of Major Professor and/or Committee Form. Use this form to have your major professor and/or your committee appointed.
• Study Plan. Submit your plan on VandalWeb.
• Committee Change Form. Use this form to change your committee membership.
• Application for Advanced Degree
• Request to Walk Through Commencement

• Confidentiality of Records
• Transcript Request 
• Application for Non-degree Admission. Use this form to apply for admission as a non-degree seeking student.
• Application for Readmission (Non-degree) . This form is for non-degree students who have not taken a course during the past 2 years.
